Annuity Loan Summary of Material Modification
Anthem mailed new ID cards on 12/22/2021
Anthem mailed new ID cards on 12/22/2021 for all active and non-Medicare plan participants effective 1/1/2022. The Group, Plan, and ID Numbers have changed. This transition is due to compliance with the No Surprises/Transparency Act as well as an eligibility platform change initiated by Anthem for our group. The benefits have not changed, but you must use the new card on and after 1/1/2022 or claims will be denied by Anthem.
You can see your new ID card at anthem.com or by using the Anthem’s Sydney Health smartphone app.
BENEFIT TRUST
To protect our members, a Health and Welfare plan has been made available by the Board of Trustees and is administered by the Iron Workers District Council of Southern Ohio and Vicinity Trust Office. Coverage includes Medical Insurance, Prescription Drug Benefits, a Healthcare Reimbursement Account, Dental and Vision Benefits, and more.
PENSION TRUST
The Iron Workers District Council Pension Plan is designed to provide some of your retirement. Your pension benefit can be a significant part of your retirement income. The amount of your pension benefit is generally based on the number of years you work for an Employer that contributes to the Plan on your behalf. Generally, the longer you work for a Contributing Employer, the greater your pension.
ANNUITY TRUST
The Iron Workers District Council Annuity Trust is designed to supplement your other retirement benefits. When you become a participant in the Annuity Plan, an individual account is established in your name. Employer contributions are made on your behalf to your individual account. You choose how to invest the employer contributions made to the Annuity Plan.
